Birth–5

Child Services

Our goal is to empower parents and families as well as children.

We’ll assess your infant or child for early intervention and guide you through the eligibility process. Our staff will talk to you about your challenges and goals and develop a unique plan to connect you with the right services for your child. We’ll help you navigate the system and monitor your child’s progress along the way. To get started now, call us at (937) 548-9057.

Eligibility For Birth-5

All referrals for children birth to age three are made through Darke County Early Intervention. Darke DD works in partnership with Darke County Health Department to provide developmental evaluations to children who have a suspected delay or who have a medical condition which has a high probability of resulting in a developmental delay. Once the Darke Early Intervention team completes the home-based evaluation and determines your child is eligible for services, you will be given options regarding community supports from which to choose. One of the choices for parents is EARLY INTERVENTION services through Darke DD. Children will have their eligibility re-determined at age three for continued services.

Children age three to five are eligible for Darke DD service coordination if they have at least two documented developmental delays reported in the Multi-Factored Evaluation (MFE) or a medical disorder that is known to cause delays. Children will have their eligibility re-determined at age six for continued services.

Next Steps

If you are eligible, you will be connected with a Service & Support Administrator (SSA) to begin your person-centered planning process, identifying service needs and outcomes for the future.

If you are not eligible for services, Darke DD will assist you in identifying alternative community services that may be available to help meet your needs.
